Those who will need additional housing assistance from FEMA are asked to remember to fill out an important form.
The Federal Emergency Management Agency sent out a "declaration for continuing need for rental assistance" form 15 days after residents received their initial rental assistance grant. Residents who will continue to need rental assistance must complete the form and send it back to FEMA.
The form helps FEMA provide additional assistance to those who are still displaced after the storm.
Those who don't have the form, or have any questions can contact FEMA by calling 1–800–621–FEMA.
